Failure to upload the required files – or submitting content without completing the scholarship form on the previous page – may result in us being unable to process your submission.

What to Submit

  • Video

    A 2-5 minute video on one of these topics:

    • What is your greatest strength and weakness?

    • Tell us about your favorite book from childhood and your favorite book now.

    • Describe the biggest challenge you’ve ever faced and how it prepared you for the future.

  • Essay

    One page, double-spaced essay describing what sparked your interest in dentistry or dental hygiene.

  • Letter of Recommendation

  • A Recent Photo

    Please submit a recent photo of yourself that we may post to social media.

  • Media Release Form

Make sure your name is included and legible on all content. Where possible, include your name in the filenames of files submitted. For example, here’s the release form:
